Acnathosis in 9 yr old

A child aged 9 came with issue of black acanthosis behind neck. She was fatigued all the time. Her glucose was lowish at 75 fasting and fasting insulin was high. Thyroid and cortisol were normal. Started her on akkermensia and leptin resistance; after 2 weeks the acanthosis has visibly improved and her fatigue has gone.
